Abstract

Background: Hypertension is a medical condition characterized by increased blood pressure in the arteries and is often referred to as the “silent killer” because it can cause sudden death without obvious symptoms. The Bukittinggi City Health Office noted that hypertension is the most prevalent disease among the people of Bukittinggi City, with the highest prevalence found in the Nilam Sari Community Health Center working area at 18.68%. Objective: This study aims to determine the relationship between family support and self-efficacy with medication adherence in hypertensive patients in the working area of the Nilam Sari Community Health Center in Bukittinggi City in 2025. Methods: This study used a quantitative design with a cross-sectional approach. The study population consisted of hypertensive patients in the Nilam Sari Community Health Center working area, with a sample size of 106 respondents selected using accidental sampling. Data were collected through questionnaires. Results: A total of 56.6% of respondents received family support, 50.9% had high self-efficacy, and 51.9% were compliant in taking medication. The results of the analysis using the chi-square test showed a significant relationship between family support (p value < 0.0001) and self-efficacy (p value = 0.001) with medication adherence in hypertensive patients. Conclusion & recommendations: There is a relationship between family support and self-efficacy with medication adherence in hypertensive patients. Researchers are advised to conduct further studies exploring other factors that may influence medication adherence, such as demographic variables and other psychological and social factors.
